Nicotine in Hookah: Exposure, Heat, and Filtration Limits

Hookah smoke exposes users to nicotine, carbon monoxide, fine particles, and other harmful substances. Heat management affects equipment performance; it does not make smoking safe or establish reduced health risk.

Shisha made from tobacco contains nicotine, and hookah smoke delivers it. The CDC reports that one hookah session can expose a person to about 1.7 times the nicotine and nearly 9 times the carbon monoxide of a single cigarette [2]. The CDC also states that the water in a hookah does not filter smoke [2]. A heat management device changes how charcoal heat reaches the shisha. Kaloud makes no claim that it changes nicotine or carbon monoxide exposure.

What is in hookah smoke?
Hookah smoke carries what comes off the heated shisha and what comes off the burning charcoal.
- Nicotine. It comes from the tobacco leaf. The CDC describes it as the same highly addictive chemical found in other tobacco products [2].
- Carbon monoxide, metals and cancer-causing chemicals. The CDC notes that the charcoal used to heat hookah tobacco can produce high levels of all three [2].
- Other toxic agents. The CDC reports that smoke still has high levels of toxic agents after it passes through the water [2].
How much nicotine does a hookah session deliver?
The CDC compares one hookah session with one cigarette. A typical session lasts about an hour [2].

A 2016 systematic review and meta-analysis pooled published studies of waterpipe and cigarette smoke. It found higher levels of nicotine, tar and carbon monoxide per waterpipe session than per cigarette [3]. Exposure depends on how much smoke is inhaled and for how long [2][3].
Does herbal shisha contain nicotine?
Shisha made without tobacco is sold as a nicotine-free alternative. The charcoal and the smoke remain. The CDC reports that studies of tobacco-based and herbal shisha found carbon monoxide and other toxic agents in the smoke from both [2].
Does the water in a hookah filter nicotine?

No. The CDC states that the water in a hookah does not filter smoke. Even after the smoke passes through the water, it has high levels of toxic agents [2]. A smoother draw is not evidence of lower exposure.
Does heat management change nicotine exposure?
Kaloud makes no claim that it does. A hookah heat management device is hardware. It holds the charcoal above the shisha and spreads its heat through metal instead of foil. The charcoal still burns, and the CDC notes that hookah charcoal can produce high levels of carbon monoxide [2].
What heat management controls is temperature. Kaloud works to a shisha range of 130 to 220°C. Above about 220°C, the glycerol and propylene glycol in shisha start to break down. A 2017 PLOS One lab study of heated glycerol and propylene glycol, the solvents in e-cigarette liquids, detected carbonyl formation starting around 215°C [1]. How hot does the bowl get in Kaloud's tests?
Kaloud logged three full sessions with a Lotus I+3 on a tall black bowl with 30 g of shisha. Two Type K thermocouples fed a LOG200-TC logger.

The hottest reading was 219.2°C. Every session stayed below 220°C at its hottest point [4]. These readings describe bowl temperature. They do not measure nicotine, carbon monoxide or any other compound in the smoke.
